Along with photo ID, you will also have to provide your address proof along with your application. Attach one of these documents to your form:

Post office passbook containing the address of the applicant
Latest Property Tax Assessment Order
Domicile certificate issued by the government
Allotment Letter of Accommodation issued by the Central or State Government (not older than 3 years)

Original certificate issued by the company

You can also use these documents, but they should not be more than three months old.

3. Date of Birth Certificate
Any one of the following documents can be produced as a date of birth certificate:

4. Photo
